Forms for Graduate Students

The JTC Department and the Graduate School require certain forms to be completed and filed at various stages of your program of study. The following list is provided for your convenience but may not be the most up-to-date. Please refer to the Graduate School’s Graduate and Professional Bulletin, forms page, and deadlines page for the latest information.

Departmental Forms

Graduate School Forms

  • GS6: Program of Study.
  • GS6 Program of Study Worksheet.
    The Program of Study, or GS-6 Form, formally lists your course work for the master's degree and identifies your graduate committee members.  You must prepare an electronic copy of the GS-6, print a hard copy, obtain signatures, and file it with the Graduate School no later than the end of your second semester, or before completing 12 credits of graduate work in residence.  (See below for the URLs for the current information and forms.) First, print a copy of the Program of Study Worksheet, fill it out, then fill out the GS6, print a copy, obtain the needed signatures, and then file the signed copy with the Graduate School.
  • GS9A: Petition for Change in Committee.
  • GS24: Report of Final Examination Results. This form must be submitted to the Graduate School, with all signatures, within 48 hours of your thesis defense.
  • GS25: Application for Graduation.
