CSREES 2008 Assurance Statement - Instructions - Web Input

• Log in to the CRISFRMS website, following the directions given for the AD416.

• Select the form to work on:     CSREES 2008   and [Proceed].

• Enter the following information:

  Check appropriate statements, supplying additional information when necessary:

  1. Biosafety of Recombinant DNA
  2. Care and Use of Animals
  3. Protection of Human Subjects

  If your project involves any of the areas of concern listed above, please contact the Research Integrity & Compliance Review Office (491-1553) and file the appropriate forms.  The approval process may take several months, so please contact them early. We need a copy of their approval before we will forward your project to USDA.

• When all information is entered, [Check Data] and note any errors. View the displayed information carefully.  Corrections or additional edits can be made by using the [BACK] button on your browser toolbar. After each edit, [Check Data] again.

• When the information is correct and complete, [Save Information]. You should see a Confirmation Screen if your information was successfully saved in the work file. You may want to print a copy of this page to keep for your records.
